DESCRIPTION:Important: This training will be offered on two dates (1/29 & 2/4). Please register for only one of the two events.\n*The information presented will be the same on both dates but locations are different* \nJoin us for this free in-person training on Wednesday\, January 29th\, 2025. Attendance is limited to 40 attendees so be sure to RSVP below to confirm your spot! \nBy the end of this in-person workshop\, participants will:\n– Gain an Understanding of Social Determinants of Health\n– Learn what a Disparity Impact Statement (DIS) is and how to Incorporate one into your prevention efforts\n– Review the National Culturally and Linguistically Appropriate Services Standards (CLAS)\n– Be Provided a Framework for Participants to Develop a Disparity Reduction Quality Improvement Plan\n– Apply Health Equity Questions and Data Across the 5 Strategic Prevention Framework Steps\n– Receive a Health Equity Matrix Template and other Tools and Resources \nThis training is eligible for 2.5 CEUs upon completion. \nLocation:\nHilton Garden Inn.\n1181 Barnes Road\nWallingford\, CT. 06492 \nQuestions?
